About questionnaires in case-taking, mine is pretty basic. Name, address, reason for consulting, medication, present and past... but most importantly to me, I ask about present AND past illnesses.
Past Illness and Disease
I write a life line of all the past illnesses, and take the time in the 2 to 3 hour consultation to see if they appear right after some event. Then I ask the patient how he/she experienced the event. That tells me about what the patient's vital energy is deeply sensitive to. THIS is a wonderful key in my practice. I find this simply amazing. Even with kids. In 90 % of the cases, on Monday the patient goes through some stress, on Monday night or Tuesday, they are sick. Even in common colds! Many times last winter, simple colds led me to the similimum of the person.
Patience is a virtue for the Homeopath
Sometimes I need to give the patient some time to remember, for they forget what they went through 10 or 20 years ago. But I find that it comes back pretty quickly, AND, the same sensitivity is often the one that triggers an illness at 10- 25- 50 years old. In a different context, the same kind of sensitivity, related to the same remedy, expressed in a different way.
So in that sense, the questionnaire is very important to me. I ask my patients to fill it out prior the consultation, so they can try to remember the month and year they developped such and such illness. You can find the questionnaire on my website. (HOMEOPATHE.ca)
Emotional Causation - Etiology
I recently saw a man who developed an myalgic encephalitis many years ago, after he bought a house and got into some legal problems. Every time he got sick in his life, it was right after some legal or financial problem that made him feel, he describes, like he was losing his power, his greatness, his importance. Besides having warm feet at night, nothing much was indicating sulfur in his personality, but the etiology, and the way HE described his experience was very sulfur-like. And that's what cured him.
Conclusion: Find Out When Problems Began
The life-line and the etiology led me to his remedy. So the questionnaire has something major for me. Asking people about past and present illnesses in order to find out what happened in their lives JUST before the got ill.
This trick I learned from my wonderful teachers, Linda Arbour and Georges Arragon, and it helps me save SOOOO much time in a treatment.
