Definition and epidemiology
Hypertension is elevated blood pressure (BP) that increases the risk of injury in target organs, which can be a heart, brain, kidney or retina. Hypertension is the condition for which are prescribed more drugs in the United States it is estimated that in America there are approximately 50 million hypertensive patients, if hypertension is defined as a value of BP greater than 140/90 mm Hg. However, only 35% of them are aware of the disease. Hypertension afflicts only 20% of American whites and 30% of the black population over 18 years old, about 90% of cases are type of essential hypertension (Cause unknown and strong hereditary predisposition) .
The rest suffer from hypertension secondary to renal parenchymal disease, renovascular disease, pheochromocytoma, Cushing syndrome, primary aldosteronism and coarctation of the aorta. The prevalence of morbidity and mortality associated with hypertension, including atherosclerotic cardiovascular disease, heart failure and renal isuficiencia increases with elevated levels of systolic and diastolic BP. It is very important to normalize the BP as this reduces cardiovascular and cerebrovascular complications.
We talk of accelerated malignant hypertension when BP increases quickly and significantly above baseline values, causing acute injury in the vessels of the retina, irrespective of the absolute number of BP. It is here where I believe the homeopathic physician must assess the risk factor of time to consider when a patient with malignant hypertension should be sent to hospital for the normalization of BP.
A number of days ago, I had a case of malignant hypertension which I could not stabilize with homeopathy. At the hospital, was detected renal artery stenosis (mechanical problem), which was quickly corrected through angioplasty. Always having the wisdom of sending a patient to hospital when not achieved the success with the homeopathic intended for, you can save the life of the patient.
We should never consider that a person is hypertensive unless the BP is greater than 210/120 mm Hg or be accompanied by symptoms affecting a target organ.

Once the patient have been diagnosed as hypertensive then we should investigate about cholesterol and triglycerides numbers, urine general exam, , Blood chemical, Thorax radiography, and ECG , even an echocardiogram. Some patients is necessary assess the cardiac function or left ventricular hypertrophy*.

Pathognomonic symptoms of hypertension**:
**Pathognomonic symptoms of hypertension are not always present…Many times the patient does not feel any symptom, that is why is called a “silent disease”, but the homeopaths know that there are always symptoms that will be helpful in selecting an appropriate remedy.

Other aspect that has to be considered as a symptom very frequent but not always detected is the symptom of the memory when the blood pressure is high:

Treatment goals should be to: reduce stress, reduce fat intake, eliminating fat intake, stop smoking and cholesterol control, loss weight.
As personal experience, in the treatment of hypertension we should consider that the most of patients on first time coming to our office they come with established treatments in allopathic field. That is the why I never suddenly stop treatment that the patient is taking, I do it in a gradual process as much as possible. In the case of hypertension the percentage of patients that reaches not to need medicine is approximately 50%. (Only homeopathic remedy).
When I'm who makes the diagnosis, generally I asses the Time- risk factor and I give adequate time to act the remedy to check what will be the answer to my patient. Obviously, the result it will depend on the skill of the homeopath, the sensitivity of the patient, the obstacles which impede the efficient operation of the remedy. (organon 3, 4, 259, 260, 261)
This obliges me to set some parameters:
1.-Case of mild hypertension (140-159 in Systolic number, 90-99 in diastolic number**)* without collateral risk factors (diabetes, heart disease, hyperlipidemia, previous myocardial infarction): Only homeopathy, as long as necessary.
2.-Mild Hypertension with one or two aggregated factors: Homeopathy with stabilization of the other factors as long as necessary.
3.-Moderate hypertension (160-179 systolic, 100-109 diastolic**)* without collateral risk factors: Only homeopathy with blood pressure monitoring every third day for 2 weeks. If patient no improvement, the patient must be controlled conventionally, later homeopathy as much as possible.
4.-Moderate hypertension with collateral risk factors: Only Homeopathy blood pressure monitoring every third day for a week with stabilization of the other factors. If patient does not improve, the patient must be controlled conventionally, later homeopathy as much as possible.
5.-Severe hypertension (180-209 systolic, 110-119 diastolic**)* without collateral risk factors: Deal with homeopathy for a few hours (2-6 hours), plus system, if there is no response the patient must be sent to hospital.
6.-Severe hypertension with collateral risk factors or is accompanied by symptoms affecting a target organ: Send your patient immediately to Hospital (if he does not respond to homeopathic emergency treatment.)
7.-Malignant hypertension or very severe (>210 systolic, >120 diastolic**)*: Send your patient immediately to Hospital (if he does not respond to homeopathic emergency treatment.)

The problem is what is the best for the patient ? Of course to find the indicated remedy and to get an immediate amelioration. If patients come to us which are under allopathic medication it is only reduced with progressing amelioration from homeopathic treatment. So the options are always, only to treat them homeopathically - no matter how severe his condition is (many homeopathic hospitals in India work in this way), or to give allopathic and homeopathic medication and reduce allopathic drugs with amelioration.
The following remedies have been found as practical and helpful in my medical practice. This does not mean that they are the only one for hypertension. Every homeopath must find the similar remedy for each patient according the symptomatic totality, it means, following the Similar´s law.
