Sorry to reply so late.
I have trated some cases of fistula ano. Different remedies were needed such as Silicia, Hepar sulf, Calcarea Sulf, Merc sol etc along with Constitutional and miasmatic interurrents. In Homeopathy the name of the disease can only point to a group of remedie from which one has to be selected after taking into account the symptoms of the patient. It is dangerous to get treated by a remedy suggested by email. There is the question of not only selecting the suitable remedy but also its potency and repetition as well as auxillary methods. The best course is to be treated by an experienced homeopath using classical homeopathy.

Please varify about the thermals of the patient,If patient is chilly ,then in my view the remedy you think Silicea, is best.If patient is hot then you may think about about acid flour.Potency in my view is 200, is best.
